Get your dates in writing
Ask your current employer when coverage ends and your next employer when coverage can begin. Keep the relevant plan documents, notices, and contact information together. Check arrangements for family members covered by the plan.
List the options to investigate
HealthCare.gov describes Marketplace plans and possible COBRA continuation coverage after losing job-based insurance. Check the current official guidance and ask your employer which options apply. Costs, enrollment periods, and eligibility depend on your situation.
Compare more than the premium
For each option, record the monthly premium, deductible, out-of-pocket limit, provider network, and prescription coverage. Verify details directly with the plan before relying on them. Add dates for any enrollment decisions and required documents.
Make the next call concrete
Try: “My current coverage ends on [date]. What do I need to do to avoid an unexpected gap, and when would this option start?” Check timing promptly with your employer, plan administrator, or the Marketplace. This planner does not establish eligibility or enroll you.
