Information you choose to send
The launch-list form collects your email address, chosen interest, consent, and submission date so Black Health Wealth can manage launch updates. It is an interest list; a weekly email service and confirmation emails are not active yet. Contact forms collect your email, inquiry type, message, and submission date so the publisher can respond. Do not send health records, insurance IDs, account numbers, or other sensitive personal details.
Where submissions go
Forms are stored in the publisher’s Cloudflare D1 database. Cloudflare hosts the site and processes requests to deliver it and protect it from abuse. No advertising pixels or third-party analytics scripts are installed in this launch build.
Local interactions
Printable worksheets and on-page checklists do not submit your entries. Check marks are kept only while that page remains open. The CMS uses essential session cookies for editor sign-in.
Your choices
Joining the launch list is optional. To request removal or ask about your information, use the privacy request option on the contact page. Provide only the email address needed to identify the record. Any future newsletter must include an unsubscribe route before sending begins.
Retention and access
Submissions are available to the publisher through its Cloudflare account. The publisher should delete records when their purpose ends or in response to a verified removal request, subject to any necessary recordkeeping. Information is not posted publicly or sold through this site.
