Ask the billing office for a breakdown
Contact the billing office listed on your provider statement and request an itemized bill for the relevant date of care. Ask for individual services, quantities, charges and any billing codes used. You can also ask for a payment and adjustment history to explain the remaining balance. The office can tell you its process, delivery options and expected response time.
Have the right details ready
Keep the provider name, date of service and bill reference in your private records. Use the provider’s secure portal or its confirmed correspondence process. If calling on behalf of a family member, ask what permission the office requires. Do not send health details or identification documents through an unverified address.
Use the request below
Adapt the sample to your situation and send it through the provider’s confirmed channel. It requests information; it does not allege that the bill is wrong, promise a lower balance or replace a formal appeal. Keep a copy and note when and where you sent it.
Compare the response with your records
Match the patient, date and services. Check for an item you do not recognize, a possible duplicate or a missing payment. If you used insurance, compare the latest EOB and ask the insurer about unexplained differences. A total charge, an allowed amount and a patient balance can refer to different parts of the claim.
Follow up with one clear question
If something remains unclear, identify the exact line and ask the office to explain it. Request a reference number and the next follow-up date. Ask whether any account activity or deadline changes during review; do not assume the request stops payment, appeal or collection deadlines.
Sample itemized-bill request
Copy the blank sample, replace the placeholders in your own document and use the provider’s confirmed secure channel. Nothing you copy or write is sent to us.
